When adding adding comments to lines in the diff, you click "Save" for each
comment, and when you're ready to submit the review, you click "Publish".
The way to reply to those comments is inconsistent. In that case, people
click "Save" for each comment, and then at the top they're supposed to
click a different "Save" button. Why isn't that also called "Publish"?
In a similar vein, it'd be nice if there were a "Publish" button at the top
of the diff view.
