496: Each diff upload should permit an associated description

Each time a diff is uploaded to a review, there should be a means of adding
a small description of the change made by the diff.  (Additionally, the
time the diff is uploaded should be recorded somewhere by reviewboard).

Then, when viewing the diffs of a review, this additional information
should be included/displayed (description and date posted) for each diff
revision.
