Sometimes hitting the Publish button takes a little while to hide the
window and it is very tempting to hit it again to see if it did anything
the previous time. This causes multiple identical reviews to be published.
To fix this, I think you should disable the Publish button as soon as it
is pressed to show them that their click was indeed processed. Not sure if
this idea sucks or not, but you could also change the button's text to
something like "Working..." to show them why the button is disabled.
What operating system are you using? What browser?
