3383: Upload file form doesn't detect errors

What version are you running?
2.0.1

What's the URL of the page containing the problem?
/r/ABCD/ (e.g. reviews.reviewboard.org/r/5824/)

What steps will reproduce the problem?
1. Click Update -> Add File
2. Click "Upload"

What is the expected output? What do you see instead?
Error message is presented. Instead nothing happens and the page is broken. In console you can see "Uncaught TypeError: Cannot read property 'msg' of undefined "

What operating system are you using? What browser?
Chrome on OS X.
