336: I can't respond 'generally' to a code review.

When I respond to a code review, I can only respond to the reviewer's
comments specifically. It would be nice to also be able to have a general
response to the entire review, much like you can have a general code review
without responding specifically to lines of code.

You should be able to add a comment at the very top of the review. (Unless we
introduced a bug and no longer show that unless they've added a comment to the top of
the review). If so, we should fix that.

I think that that's what happened - that you can only do it if the reviewer had a top
level comment as well.
