2094: Account registration page does not register new accounts

What's the URL of the page containing the problem?

What steps will reproduce the problem?
1. Go to account registration page
2. Fill out registration fields
3. Hit 'Register'

What is the expected output? What do you see instead?
Expect to get registered for an account, instead no change occurs

What operating system are you using? What browser?
Tested on Mac OSX in all of Firefox 3.6/Chrome 11.0.696.57/Safari 5
