Note the fields are blank. We had a user who hadn't noticed there was a network problem and then reported this error.
What operating system are you using? What browser?

Please provide any additional information below.
Not sure if this is a bug of enhancement, I think updating the error text/form to add some text along the lines of "check that the reviewboard server is running and accessible" as well as any (http) errors. For some reason there was no error information at all in the form.
